Qualification Criteria
- Patients between 18 and 59 years of age.
- Body Mass Index (BMI) of 27 or greater.
- In-person provider visit required every 3 months.
Ineligible Conditions
- Age under 18 or 60 and older.
- Chest Pain, Heart Disease, Stroke, or Arrhythmia.
- Uncontrolled High Blood Pressure or Glaucoma.
- Cancer (active or recent treatment).
- Anxiety Disorder.
- Severe Anemia.
- BMI reaches ideal body weight or BMI < 21.
Important Practice Policies
• Primary Care Deferred: We manage weight control exclusively. Blood pressure, diabetes, and acute medical management remain with your PCP.
• Insurance & Medicare: Cash-based practice. We do not file insurance claims and are opted out of Medicare.
• Deposit: A $75 non-refundable deposit holds your initial spot and applies directly toward your first visit fee.
